Triage
Safety, active-location, discontinued-service, private-address, payer, rate, and licensing conflicts receive priority review. Content may be temporarily removed while the responsible owner verifies the fact.
Correction record
Material corrections update the page date and, when useful to readers, include a concise correction note. Minor spelling, formatting, and accessibility fixes may be made without a public note.
No silent substitution
A disputed claim is not replaced with another unverified claim. When current evidence is incomplete, the page states the limitation or withholds the field until approval.
