Job Description
Under moderate-to-general supervision, the School-Based Support Mentor works independently to support clients in a positive, school-based behavioral care environment. This role involves modeling healthy behaviors and supporting clients in achieving treatment goals related to mental health, independence, socialization, and community engagement.
Essential Job Functions
- Conduct assessments of functional living skills and assist in developing individualized treatment/service plans.
- Monitor client progress in independent living skills and community support integration.
- Collaborate with school staff, families, landlords, employers, and transportation providers.
- Provide crisis support in alignment with treatment/service plans.
- Share relevant client updates with internal teams and authorized external providers to ensure continuity of care.
- Complete required documentation and progress notes promptly and accurately.
- Maintain confidentiality of protected health information in accordance with Crimson Heights procedures and applicable laws.
- Participate in all staff trainings, team meetings, and service planning sessions.
Independent Living Skill Areas May Include:
- Social Skills: Participating in community-based leisure activities.
- Executive Functioning: Managing money, hygiene, personal safety, and setting educational and vocational goals.
- Communication Skills: Appointment-setting, assertiveness, stress management, and behavior regulation techniques.
- Community Access: Navigating transportation systems, job placement, school attendance, and community resources.
Education & Experience
- Required: High school diploma and 6–12 months of experience working with individuals with autism, oppositional defiance disorder (ODD), or conduct disorder.
- Preferred: Associate’s degree or higher in psychology, sociology, social work, human services, or another behavioral health-related field. Training in behavior analysis is a plus.
